I saw that!!  that was good!!  Reminded me somewhat of AngelHeart–another psychological thriller along the same line (with a different plot) made in the 80’s with Mickey O’Rourke and that girl (I can’t remember her name) who Played Bill Cosby’s oldest daughter in the Cosby show–then she was in the spin off A Different World–she was the female in this move.  it was pretty good. cogge`

Jacobs Ladder has a very unexpected ending and that  has Tim Robbins in it. Post-Vietnam war and some of his PTSD type stuff and flash backs to the war stuff and sometimes you wonder what is real and what is in his head and eventually there is a surprise ending. cogge`

This is totally off the mark, but I just saw KPAX, and that movie had two endings. You can pick the ending you like, and best of all it does not glorify mental illness, like some other films <ugh, no names mentioned It was one of the best films I have seen, and the soundtrack was equally stunning.
